To save time and make the registration process go
as smoothly as possible, please arrive 10 minutes before your
appointment time.
Before your visit, please follow these steps:
- Download
your New Client Registration Form Here.
- Please provide us with your prior
veterinarian's information so we can request records in advance for
good continuity of care.
- Please bring any records you may have received
with your pet, especially newly adopted or purchased pets.
- Please bring a fresh stool sample to your
appointment if one has not been tested within the past 12 months.
